What You’ll Do

Confidential Client is hiring a Case Manager to join their residential Substance Use Disorder (SUD) services team in Chicopee, MA. You will provide direct case management, advocacy, and recovery support to residents, working with the Program Director/Clinical Supervisor and Recovery Specialists to promote safety, treatment engagement, and successful transitions to community-based care.

Key responsibilities:

• Provide advocacy and case management services for assigned residents
• Complete clinical documentation: assessments, individual action plans, group and weekly progress notes, discharge and aftercare planning
• Facilitate evidence-based psychoeducation and recovery groups
• Conduct weekly individual case management sessions
• Administer and document drug/alcohol screens and EIM/ESM forms as required by funders
• Maintain supportive, therapeutic contact with residents; use de-escalation techniques when needed
• Coordinate transportation and appointments when necessary
• Participate in case consultations, staff meetings, and trainings
• Maintain working relationships with community social service agencies
• Support adherence to program and agency policies; may monitor medications or complete hourly checks as needed

Minimum Qualifications

• Associate’s degree plus 2 years’ relevant experience (Bachelor’s degree preferred; 1–2 years’ experience preferred)
• Experience providing substance use services or working in behavioral health/residential treatment
• Strong written and verbal communication skills, including conflict resolution
• Solid organizational skills and ability to manage multiple priorities
• Demonstrated ability to lead by example and work collaboratively with interdisciplinary teams
• Valid driver’s license and reliable access to an automobile; must meet agency driving-history requirements
• Bilingual candidates encouraged to apply

• Experience facilitating evidence-based groups (e.g., relapse prevention, psychoeducation)
• Familiarity with EIM/ESM and funding-source documentation requirements
• Clinical licensing support interest or a plan to pursue licensure
