What You’ll Do
Confidential Client is seeking a Case Manager — Behavioral Health to provide assessment, care coordination, and ongoing support for patients with behavioral health needs. You will work with interdisciplinary teams to address social determinants of health, reinforce treatment plans, and support continuity of care.
- Manage an assigned caseload and screen for common mental health and substance use disorders.
- Assess social determinants of health and develop patient-centered care plans.
- Provide culturally competent, strength-based, recovery-oriented case management through outreach, education, and family support.
- Coordinate internal and external referrals (behavioral health services, substance use treatment, housing assistance, vocational rehabilitation, etc.).
- Reinforce clinical recommendations, medication adherence, lab completion, nutrition guidance, and referral follow-through.
- Conduct in-person and virtual patient check-ins, including regular contact for patients on active safety plans.
- Use crisis intervention skills to help prevent or de-escalate crises and promptly report safety concerns.
- Maintain timely, accurate documentation in the EHR, including case notes, care plans, assessments, and outcome measures.
- Track caseload activity and clinical outcomes; complete required reports and quarterly patient summaries.
- Participate in caseload consultations and collaborate with providers, therapists, families, and community partners while complying with HIPAA and applicable regulations.
Qualifications
- Bachelor’s degree in social work, psychology, human services, or related field from an accredited four-year college or university.
- Experience with behavioral health case management, care coordination, or related roles preferred.
- Strong interpersonal skills and cultural competence working with diverse patient populations.
- Comfortable performing both in-person and virtual outreach and follow-up.
- Demonstrated ability to use EHR systems, document accurately, and meet reporting deadlines.
- Ability to manage multiple priorities, maintain confidentiality, and work collaboratively on interdisciplinary teams.
Licenses, Certification, or Registration
- Certified Case Manager preferred but not required.
- This position requires a Level 2 background screening through the Florida Care Provider Background Screening Clearinghouse.
