Role Overview
Confidential Client (Healthcare & Nursing) is hiring a community-based Case Manager (Adults) to join their team in Taylor, MI. This role focuses on person-centered community outreach and care coordination for adults with mental illness. You will perform assessments, develop individualized plans of service, provide monthly face-to-face contacts, support crisis planning and transitions from inpatient settings, and link clients to community supports.
What You’ll Do
- Conduct comprehensive case management assessments to identify needs and service preferences
- Gather and integrate information from consumers, families, guardians, Adult Foster Care providers and medical/treatment records
- Use motivational interviewing to assess consumer readiness and goals
- Determine eligibility for entitlements and community services and connect consumers to appropriate supports
- Develop and coordinate Individualized Plans of Service with consumers and treatment teams (person-centered planning, IDDT, self-determination, Medicaid guidelines)
- Provide face-to-face interventions at least monthly and conduct home visits and community outreach as needed
- Lead skill-building interventions individually and in groups per service plans
- Maintain regular communication with treatment team members and external providers to monitor care delivery
- Reassess service needs at least every six months and update plans accordingly
- Coordinate primary and specialty healthcare providers to ensure continuity of care
- Assist with crisis intervention, discharge planning and follow-up services after hospitalization
- Maintain and update a community resource file and establish collaborative relationships with local providers and organizations
- Document all face-to-face activity and phone contacts, complete required documentation on time, and respond to chart reviews and audits
- Participate in clinical supervision, case consultations and team meetings
- Meet productivity standards and complete required program/position training hours annually
- Perform other duties as assigned
Qualifications
- Bachelor’s degree in a human services field (minimum)
- Must obtain Qualified Mental Health Professional (QMHP) credential within 120 days of hire
- At least one year of experience (can include fieldwork or internship) working with adults diagnosed with mental illness
- Valid Michigan Chauffeur License and a safe driving record (required for community-based visits)
- Strong skills engaging adult mental health consumers, families and supporters
- Experience developing individualized plans and connecting consumers to community resources
- Orientation toward community outreach and ability to work independently
- Demonstrated cultural competency, nonjudgmental approach, and capacity to handle crisis situations appropriately
- Effective organizational, verbal and written communication skills
- Maintain appropriate professional licensure/certification and complete 25 hours of program/position-specific training annually
Compensation & Benefits
- Estimated salary: $46,352 per year (estimated)
- Salary messaging in source materials notes ranges start at $25.82 per hour; actual pay will vary based on transferable experience and qualifications
