What You’ll Do

The Care Coordinator will support a perinatal behavioral health program (PROMISE) by improving access to care, coordinating referrals and appointments, and maintaining accurate program and clinical data. Responsibilities include:

  • Serve as a primary point of contact for PROMISE patients — handle referrals, scheduling, forms, resources, and access to services.
  • Manage referrals end-to-end: receipt, scheduling, outreach, follow-up with patients and referring providers.
  • Provide community resource information and help patients navigate PROMISE and other behavioral health services.
  • Schedule and reschedule appointments in Epic and conduct outreach regarding missed visits or incomplete paperwork.
  • Document referral and care-coordination encounters in Epic, REDCap, Excel, and other approved systems in accordance with program and institutional requirements.
  • Maintain patient confidentiality and promptly route clinical questions, safety concerns, medication questions, and treatment requests to licensed clinicians.
  • Maintain relationships with referral sources and support outreach to referring practices and community partners.
  • Track key patient and program metrics (e.g., census, six‑month postpartum status, patients needing clinician review) and collect/compile clinical and program data for grant reporting, productivity, evaluation, and quality improvement.
  • Support daily program operations, process improvement, and administrative tasks for PROMISE program activities and team functions.

Who You Are / Key Skills

  • Comfortable with care coordination, referral management, patient navigation, and follow-up.
  • Able to engage professionally and compassionately with pregnant and postpartum patients experiencing mental health concerns.
  • Strong time-management, organization, and attention-to-detail skills.
  • Able to recognize concerns that require clinical review and escalate promptly to licensed clinicians.
  • Effective communicator with patients, providers, office staff, and behavioral health clinicians.
  • Proficient with Microsoft Office; able to learn additional software and data-tracking systems.
  • Experience with electronic health records (Epic preferred) and data management tools (REDCap preferred).
  • Commitment to equitable, inclusive, and patient-centered care.

Minimum & Preferred Qualifications

Minimum Qualifications

  • Bachelor’s degree in Psychology, Counseling, Social Work, Public Health, or a related field from an accredited institution.
  • At least 1 year experience in healthcare systems with patient navigation, scheduling, or similar roles.
  • Authorized to work in the U.S. without sponsorship (sponsorship not available for this role).

Preferred Qualifications

  • Spanish language proficiency.
  • Experience with REDCap or other data management programs.
  • Experience or training in perinatal and infant mental health.
  • Experience with crisis management or triage.
  • Experience with program development, evaluation, process improvement, and Excel/data entry.
  • Experience with Epic scheduling functions.

Location & Schedule

  • Work location: Onsite (Centennial, CO).
  • Employment type: Full‑time.

Compensation & Benefits

To comply with state pay-transparency requirements, the hiring ranges for this role are provided below:

  • Entry level hiring range: $60,257 — $70,500 per year
  • Intermediate level hiring range: $65,574 — $76,721 per year

Total compensation includes the pay above plus benefits such as medical and dental plan options, disability/life/vision insurance, employer retirement contribution, generous paid time off, tuition benefits, and public transit discounts.
