Role overview

Confidential Client is hiring a Bilingual Social Worker (BSW) to provide case management and resource navigation for Spanish-speaking pediatric patients and families at clinics serving southern Salt Lake County. This is an entry-level social work role under general supervision focused on screening, care navigation, application assistance, referrals, and follow-up. Service must be delivered directly in Spanish (not through an interpreter).

What You’ll Do

  • Meet with referred patients and families (in person or by phone) to assess needs and barriers.
  • Screen for unmet needs such as insurance coverage, behavioral health, food, housing, transportation, childcare, utilities, immunizations, and school-related services.
  • Complete and submit applications with families (Medicaid, CHIP, Marketplace, WIC, SNAP, clinic assistance) and track outcomes.
  • Place and confirm referrals to community agencies and behavioral health providers.
  • Follow up on open cases on a defined schedule and maintain current knowledge of community resources and eligibility rules in the southern service area.
  • Deliver all services in Spanish, including applications, phone calls, and written follow-up.
  • Interpret during case management conversations between Spanish-speaking families and providers.
  • Adapt Spanish patient materials so they read naturally and clearly.
  • Accept referrals from clinic staff, respond within agreed turnaround times, and report status back to providers.
  • Attend clinic huddles or care-team meetings on a rotating schedule.
  • Document contacts, referrals, and applications accurately and promptly in the medical record.
  • Handle patient information in compliance with HIPAA and additional protections for behavioral health and minors.
  • Follow mandatory reporting obligations and escalate safety concerns to the provider or Director immediately.
  • Work within BSW/SSW scope of practice and refer clinical or diagnostic concerns to licensed clinicians.

Qualifications

Required

  • Bachelor of Social Work (BSW) from a CSWE-accredited program.
  • Utah Social Service Worker (SSW) license, or eligibility to obtain SSW within 90 days of hire (a CSWE-accredited BSW qualifies without exam or additional supervised hours).
  • Full professional fluency in Spanish and English (spoken and written). Fluency will be assessed at interview — this is required.
  • Valid driver’s license, reliable transportation, and current automobile insurance (travel between southern clinics required).
  • Good judgment about documentation, confidentiality, and when to involve providers or licensed therapists.

Preferred

  • Working knowledge of Medicaid, CHIP, and community assistance programs in Utah, or the ability to learn them quickly.
  • Experience in healthcare, public health, or community services settings.

To comply with US hiring laws, applicants must have the legal right to work in the United States. Reasonable accommodations are available for applicants with disabilities during the application and interview process — please request accommodations if needed.

Schedule & Work Environment

  • Part-time: 20–25 hours per week (scheduled based on clinic needs; subject to change).
  • Occasional hours outside standard clinic times may be required for family availability.
  • Clinic and office settings; patient contact in exam rooms, offices, and by telephone.
  • Regular travel between southern clinics during the workday; mileage reimbursed at the current rate.
  • This position does not include diagnosing or providing psychotherapy — clinical issues are referred to licensed clinicians.

Compensation

  • Hourly pay: $25.00 – $30.00 per hour (as listed by the source). Actual offer will align with experience and applicable laws.
