Position overview
Confidential Client is looking for a bilingual (Spanish/English) Social Worker to join their southern-clinic care team. This is an entry-level, part-time case-management role (20–25 hours/week) focused on connecting families with medical coverage, behavioral health, food, housing, transportation, and other community resources. Services are delivered directly in Spanish — not through an interpreter.
What You’ll Do
- Provide case management and navigation for referred patients and families (in person and by phone)
- Screen for unmet needs including medical coverage, behavioral health, food, housing, transportation, childcare, utilities, immunization access, and school-related services
- Complete and submit benefits/applications with families (Medicaid, CHIP, the Marketplace, WIC, SNAP, and clinic financial assistance) and track outcomes
- Place and track referrals to community agencies and behavioral health providers; confirm patients were reached
- Follow up on open cases on a set schedule and stay current on local programs and eligibility rules in the southern service area
- Deliver the full service in Spanish — applications, calls, and written follow-up — and interpret between Spanish-speaking families and providers as needed
- Adapt Spanish patient materials so they read naturally for families
- Take referrals from providers and clinic staff, respond within agreed turnaround times, and report status updates
- Attend clinic huddles or care-team meetings on a rotating schedule across southern sites
- Document every contact, referral, and application in the medical record promptly and factually
- Handle patient information in compliance with HIPAA and applicable protections for behavioral-health and minor-patient data
- Follow mandatory reporting obligations and escalate safety/clinical concerns to the provider or Director immediately
- Work within license boundaries and refer clinical issues to licensed clinicians
Qualifications
- Required:
- Bachelor of Social Work (BSW) from a CSWE‑accredited program
- Utah Social Service Worker (SSW) license, or eligibility to obtain SSW with licensure completed within 90 days of hire (a CSWE-accredited BSW qualifies for SSW with no exam or additional supervised hours)
- Full professional fluency in Spanish and English (spoken and written). Fluency will be assessed during interview — this is required
- Valid driver’s license, reliable transportation, and current automobile insurance (regular travel between southern clinics)
- Sound judgment about documentation and confidentiality; discretion with sensitive patient/family information
- Preferred:
- Working knowledge of Medicaid, CHIP, and community assistance programs in Utah, or ability to learn quickly
- Experience in healthcare, public health, or community services settings
Schedule & work environment
- 20–25 hours per week (scheduled according to clinic needs)
- Occasional hours outside standard clinic times when families cannot meet during regular hours
- Clinic and office setting; patient contact in exam rooms, offices, and by telephone
- Regular travel between southern clinic sites during the workday; mileage reimbursed at current rate
- Extended time at a workstation and on the phone
Compensation
- Pay: To be determined (posting includes an estimated compensation marker shown as $41,888 per year — estimated). Final salary or hourly rate will be provided by the client during offer/negotiation.
